We will provide four FREE fest tables and four FREE benches and four FREE chairs per vendor
space
. Each additional table will cost €4, and each additional bench will cost €2.

Notes to Vendor Committee:
If you have an existing BPA #, we just need this form and proof of PAID, current insurance.  If you do not, please send a
copy/scan of the following when you return this form:
1. Business Liability Insurance (Betreibshaftpflictversicherung) – a certificate signed by the insurer that you carry business liability
insurance in the minimum amount of €2.000.000 for public and €500.000 for bodily injury and property damage. Insurance must be
current and paid before we will issue a contract.  You must present proof of payment!
2. Copy of your Current Trade License (Gewebeschein) – this is required (no exceptions)! If you do not have this requirement, please
contact the Vendor Chair.  We will work with you to get the correct form for your country or product as deemed necessary.
3. 2012 Merchandise Price List (Preisliste) – All prices will be marked in American Dollars.
You will receive an invoice from the HCSC Bazaar Treasurer and a contract from the Contracting Office!
